Aging in place, or thriving in place, describes a person living in the residence of their choice, for as long as they are able, as they age. This includes being able to have any services (or other support) they might need over time as their needs change.

If you are living in the residence you raised your children in, there are probably boxes of toys, school papers, clothing, and memories of days gone by.  Attics may be filled to capacity or basements that are more like archaeologic digs then living space!  When aging in place, it is important to declutter, organize, and liquidate some household items that are no longer in use.

Identify Barriers In The Home To Thriving In Place

Our transition team begins by understanding your current and future healthcare needs and functional ability.  We work with physical and occupational therapists to identify possible barriers, fall risks, and potential problem areas in the home.

Create a Plan

The next step is creating a plan including tasks, timeline, and estimated costs.  Most often, our transition team can sort and declutter leaving a safe environment.  You may need a grab bar added, railing, or minor construction.  We work with a select team of professionals and vendors who have been vetted and meet our quality standards.
